DMARD Therapy During Pregnancy & Lactation Ideally all DMARDs will be discontinued before conception. However, if indications are strong, SSZ and HCQ may be used with caution. It is recommended to discontinue MTX 90 days before conception for females & males. Leflunomide has a very long half-life. For females, cholestyramine washout is recommended unless stopped >2 years before attempting conception. For males a 3-month interval after stopping treatment is recommended.
